Description
The purpose of this cooperative agreement is to promote health, wellness and the adoption of healthy behaviors with the objective of preventing and/or reducing chronic conditions associated with disability. All activities should target people with disabilities that have mobility, physical limitations, and/or intellectual disabilities (hereafter referred to as PWDs).
