The purpose of the program is to allow grantees to effectively complete objectives for heart disease and stroke prevention in DP07-704 by strengthening coordination around control of high blood pressure and high blood cholesterol, improving quality of care, and eliminating disparities, focusing on the health care and worksite settings.
