Description
This funding opportunity announcement solicits applications for a new Special Project of National Significance (SPNS) multi-site initiative entitled Building a Medical Home for Multiply Diagnosed HIV positive Homeless Populations - Evaluation and Technical Assistance Center. Under this funding opportunity announcement, a cooperative agreement will be issued to fund a single organization for up to five years at a maximum grant award of $550,000 per year. The awardee will lead a multi-site evaluation and provide technical assistance to eight demonstration sites funded under a separate announcement, Building a Medical Home for HIV positive Multiply Diagnosed Homeless Populations Initiative - Demonstration Sites (HRSA-12-100). Organizations that apply under this announcement may not apply under the Demonstration Sites announcement.
