Description

The purpose of this Indian Health Service cooperative agreement is to provide diabetes treatment and/or prevention activities and/or services (also referred to as "activities/services") for AI/AN communities. Grantees will implement one Special Diabetes Program for Indians Diabetes Best Practice (also referred to as "Best Practice") and report data on its Required Key Measure. Grantees may also implement other activities/services based on diabetes-related community needs and develop an evaluation plan. Activities/services will be aimed at reducing the risk of diabetes in at-risk individuals, providing high quality care to those with diagnosed diabetes, and/or reducing the complications of diabetes. ** Please note: Although Grants.gov will accept compressed or "zipped" files, Indian Health Service cannot. Applications submitted for IHS funding opportunities are transmitted from Grants.gov to our grants management system, and our system does not accept these files. The Attachment pages in the application package can accept as many files as you need to submit, so there is no need to package them into "zip" files. **
