Executive Summary - The Centers for Disease Control and Prevention announces availability of Fiscal Year 2016 funds for FOA EH16-1606, Comprehensive Asthma Control through Evidence-based Strategies and Public Health – Health Care Collaboration. The award’s purpose is to maximize the reach, impact, efficiency, and sustainability of comprehensive asthma control services. Comprehensive asthma care entails providing a seamless alignment of the full array of services across the public health and health care sectors so that people with asthma receive all, not just some, of the services they need. Providing comprehensive care at a population level requires a stepwise approach. The first step is to ensure the availability of and access to guidelines-based medical management and pharmacotherapy for all people with asthma. Then, for the segment of people whose asthma remains poorly controlled, additional next steps provide or link them with progressively more individualized services (e.g., intensive self-management education, home-based trigger reduction services, otherenvironmental management strategies). The FOA includes three types of strategies: infrastructure strategies to support leadership, strategic partnerships, strategic communications, surveillance, and evaluation; services strategies to expand school- and home-based services; and health systems strategies to improve coverage, delivery, quality, and use of clinical services. Expected project period outcomes include outcomes for people with asthma, such as better asthma control and quality of life, and outcomes related to health care and other supporting systems, such as increases in the provision of and coverage for comprehensive asthma control services by health care, education, housing, and other organizations as well as the coordination of health care and public health services. Awardees are also expected to make progress toward long-term outcomes, including reducing disparities in asthma outcomes and expanding the development and use of practice-based evidence about effective approaches to asthma control at the population level.
