This notice announces the opportunity to apply for funding for the Fiscal Year (FY) 2019 Health Center Controlled Networks (HCCNs). For purposes of this funding opportunity, HCCNs are groups of health centers1 working together to use health information technology (health IT) to improve operational and clinical practices. The purpose of this funding opportunity is to support health centers to leverage health IT to increase their participation in value-based care through: • Enhancing the patient and provider experiences; • Advancing interoperability; and • Using data to enhance value.
