Description

This NOFO works to prevent and control tobacco use, diabetes, and cardiovascular disease in the USAPIs, the U.S. Virgin Islands, and Puerto Rico. This funding opportunity is comprised of two components – the Core Component and an optional Oral Health Component. Core Component: This competitive component is required of all applicants and will support the implementation and evaluation of a core set of evidence-based strategies designed to address tobacco use and secondhand smoke exposure, diabetes prevention and management, and cardiovascular disease prevention and management. Applicants must propose a comprehensive work plan that addresses each of these areas in alignment with the four strategies indicated in the NOFO. Oral Health Component: This competitive component is optional and will support the implementation and evaluation of a small set of activities designed to improve oral health capacity and better address the oral health needs of Island communities. All applicants must apply for the Core Component in order to be considered for the optional Oral Health Component via a single application that clearly identifies the Core and Oral Health Components. Only successful applicants under the Core Component will be eligible for Oral Health Component funding. Final funding amounts will depend upon the quality of the application, regardless of whether an applicant intends to support implementation of the Oral Health Component.
