Description
This Funding Opportunity Announcement invites applications for projects that propose to test T4 implementation research strategies for delivering proven-effective, evidence-based community-level interventions for reducing or eliminating cardiovascular and/or pulmonary health disparities, and that promote and improve population health in high-burden communities. For the purpose of this FOA, T4 implementation research is defined as research to identify strategies to enhance sustainable uptake of proven-effective interventions in routine clinical and public health practice and community-based settings to achieve maximal population health impact.
