Description

The Centers for Disease Control and Prevention (CDC) announces the availability of funds to develop multisector action plans that address the social determinants of health (SDOH) by accelerating action in state, local, territorial, and tribal jurisdictions that lead to improved chronic health conditions among Americans experiencing health disparities and inequality.
