Description

The purpose of this Notice of Funding Opportunity Announcement (NOFO) is to assess the long-term effectiveness, cost-effectiveness, and barriers to enrollment in or completion of structured lifestyle interventions (SLI), as implemented by the National Diabetes Prevention Program (NDPP). This NOFO has two (2) components to achieve the purpose of the program: Component A: To evaluate the long-term (≥5 years) effectiveness and cost-effectiveness of SLI participants, and identify barriers to participation and adherence in these SLI programs, overall and by age group, sex, race/ethnicity, and socio-economic status. The study participants are those who meet the NDPP eligibility criteria and completed the NDPP curriculum. The study will also include a control group consisting of individuals who meet the NDPP eligibility criteria but are either not enrolled in the SLI program or do not complete it. Applicants shall connect individual level data from SLI program providers, electronic health records and claims with follow up information on the study participants for a minimum of 5 years. The study shall include SLI delivered in person, virtually, and a combination of the two. Component B: The Coordinating Center provides the infrastructure for data storage (if needed), harmonizing data collection and data analysis, logistic and communication support to enhance the efficiency, productivity for component A.
