Description
Health system interventions aimed at preventing, controlling, and managing high blood pressure and high blood cholesterol may include aligning with national guidelines and recommendations; supporting better continuity of care across health care settings; maximal use of electronic health record data; technological advances that improve medication adherence; fully utilizing community health workers, and other related interventions supporting community and clinical links and partnerships. Building on relevant advancements and lessons learned from the previous funding opportunity (CDC-RFA-DP18-1817), this Innovative Cardiovascular Health Program will build towards the next phase by focusing on partnerships to improve the overall prevalence, detection, and control of hypertension and hypercholesterolemia in priority populations and address SDoH, stress/mental health, health inequity, and injustice – all factors that can lead to and/or exacerbate CVD.
