The Centers for Disease Control and Prevention (CDC) announces the availability of funds to develop multi-sector action plans that address the social determinants of health (SDOH) by accelerating action in state, local, territorial, and tribal jurisdictions, and communities that lead to improved chronic health conditions among Americans experiencing health disparities and inequality. The award ceiling for this NOFO is $125,000.
