Real Choice Systems Change (RCSC) and Aging and Disability Resource Center/Area Agencies on Aging (ADRC) Grants for Fiscal Year 2008 CMS has available approximately $ 8 million in funding to continue support States' efforts to address complex issues in long-term care reform. In addition, CMS was also awarded $5 million for Aging and Disability Resource Center /Area Agencies on Aging grants. Base Grant Category: Development and Implementation of a Person-centered Hospital Discharge Planning Model CMS is offering three years grants in this category for awards of up to $800,000. Only one grant per State will be awarded. In addition to this base grant, two additional options are offered. States may apply for only the base grant funding, or funding under one additional option. A State can not apply for both options. This grant solicitation builds upon the successes and direction of earlier RCSC and ADRC grant opportunities by providing targeted assistance to States' in their efforts to improve hospital discharge planning through collaboration with Aging and Disability Resource Centers, Area Agency on Aging and the Centers for Independent Living. Specifically, this grant opportunity is designed to: 1. Promote the development and implementation of enhanced Person-centered Hospital Discharge Planning Models that meaningfully engage Medicaid-eligible, and potentially Medicaid-eligible individuals with disabilities (and their informal caregivers) to increase the opportunity for individuals to receive home and community-based services. 2. Increase the capacity of existing ??? or develop new-- single entry points (including ADRCs) to provide critical linkages to available long-term care services in the community and much needed supports for informal caregivers themselves; Option #1 Enhancing or Expanding Aging and Disability Resource Centers/ Single Entry Point Programs (ADRC/SEP) CMS is offering funding for this Option for three years with awards up to $500,000. A three-year grant option to States will be awarded in the amount of $300,000-$500,000 per award. In addition to developing and implementing the Person-centered Hospital Discharge Planning Model, funding may be sought through this grant opportunity for assistance in enhancing or expanding existing Aging and Disability Resource Center (ADRC) programs or other single entry point (SEP) programs. An award under this option will better position ADRCs/SEPs to play an active role in implementing the State's proposed Person-centered Hospital Discharge Planning Model. Applicants seeking funding under this option must be expanding or enhancing their ADRC/SEP projects within the framework established under the AoA and CMS ADRC Program Announcements published in 2003 and 2005. In addition, ADRC/SEP programs funded under this option must work in partnership with Area Agency on Aging, Centers for Independent Living and other entities representing specific target populations to be served. Option #2 Development of a New Aging and Disability Resource Center/ Single Entry Point Program (ADRC/SEP) CMS is offering funding for this Option for three years with awards up to $800,000. Overview In addition to developing and implementing the Person-centered Hospital Discharge Planning Model, funding may be sought through this grant opportunity for assistance in developing a new Aging and Disability Resource Center (ADRC) program or other single entry point (SEP) program. An award under this option will better position an ADRC/SEP to play an active role in implementing the State's proposed Person-centered Hospital Discharge Planning Model. SEP programs should serve as a resource for both public and private-pay individuals and serve older adults, younger individuals with disabilities, family caregivers, and persons planning for future long-term support needs. ADRC/SEP programs must partner with Area Agency on Aging, Centers for Independent Living and other entities representing specific target populations served by an ADRC/SEP.
