The purpose of this initiative is to develop family history tool for the taking of women's health history spanning their life cycle. The primary care disciplines of family practice, and obstetrics and gynecology and nursing have a duty to understand the content needed and processes involved with obtaining a family history. In addition, as our scientific understanding of the molecular and genetic/genomic basis for health and disease improves, the importance of family health history as a predictive tool has increased. This fact has been highlighted throughout HHS by the Surgeon General's online web portal for collecting family health history information, 'My Family Health Portrait' (https://familyhistory.hhs.gov/) and Health Resources and Services Administration/Maternal and Child Health Bureau's (HRSA/MCHB) efforts in developing a tool for the education of the public about their family history and genetics (http://www.geneticalliance.org/ws_display.asp?filter=ccfhh). Building upon the HRSA/MCHB tools, the Genetics in Primary Care Project (http://genes-r-us.uthscsa.edu/resources/genetics/primary_care.htm) and Genetic Tools (http://www.genetests.org/), a web based information site that provides background information, teaching cases, and links to other resources, to facilitate teaching about genetics in primary care settings, the initiative also focuses on the incorporation of identification and understanding of Red Flags or signs and symptoms found in the patient history and clinical examination, that may tie the health history to a disorder or a serious pathology. This initiative will provide models, best practices, and dissemination strategies for a health history tool for women that integrates genetics, including newborn screening information into a health history. In addition, the tool development should address the need for the development and dissemination of information and educational and decision-making materials about genetics and newborn screening to the patient (and family) at the appropriate points in their life span as well as the providers, with action activities corresponding to the red flags or signs and symptoms raised in the history taking process. As the tools are developed, the grantee also should be cognizant of the eventual incorporation of the history taking tools and accompanying information, educational and decision-making materials into electronic health records.
