Project 1: Developing a Comprehensive Child Health Profile The goal of this grant activity is to 1) make comprehensive, public and private state-level health information more available to children, their families, and their health care providers, 2) ensure that children have access to primary care services and receive coordinated, comprehensive care, and 3) enable health care providers to develop behavioral/social/health profiles that will identify children who are at risk for developing disabling conditions. It is intended that the funds be used to develop linkages among several public and private health care entities serving children with and without special health care needs. This funding will seek to develop data linkages and sharing among Indiana Regional Health Information Exchange, Indiana State Public Health Department, and Indiana State Medicaid for development of an expanded child health profile or electronic health record. An integral focus of this funding cycle involves building partnerships between these public and private concerns in order to provide a framework for an expanded health information system that is accessible to families and private health care providers. It is further intended that this funding provide an evidence base of best practices in building comprehensive, integrated child health information systems with the capacity to support the medical home as a component of an electronic health record. Project 2: Developing Electronic Vital Records The purpose of this funding activity is to assist states in implementing a web-based electronic birth records system. The timely collection and transmittal of birth records is a necessary component of an integrated child information system. Many current state vital statistics systems are based on outmoded vital registration practices and systems, causing concern about data quality, timeliness, and the lack of real-time linkage capabilities. It is intended that this funding be used to implement a conversion to electronic birth records using the 2003 Standard Birth Certificate as recommended by the National Association of Public Health Statistics and Information Systems (NAPHSIS) and the National Center for Health Statistics (NCHS) of the Centers for Disease Control and Prevention (CDC). The conversion process requires more complete automation at the level of primary data collection; building relationships among the providers of source records, the state registration offices, and the National Center for Health Statistics (NCHS); and the integration of state, local and federal business processes. It is further intended that the web-based vital record system integrate with other State-based data systems, such as newborn hearing screening, blood spot screening, and immunizations.
