Population-based Surveillance for Hemoglobinopathies

Description
Purpose: The purpose of this program is to provide support through CDC cooperative agreements for non-research activities to collect and link data on selected samples of patients for population-based estimates. This announcement is comprised of multiple funding Modules: (1) Module A: Surveillance of Hemoglobinopathies in States; (2) Module B: Capacity Building and Surveillance of Hemoglobinopathies in States with a High Historically Underserved Population. The programs in Module A and B are a pilot and designed as a learning collaborative effort between states with a mixture of characteristics.
