Description

The purpose of this initiative, Early and Continuous Screening in the Medical Home, is to establish a Center to work with pediatric health care professionals to: 1) promote awareness of the need for and benefits of early and continuous screening for congenital and heritable disorders, sensory impairments, developmental delay, autism, mental health disorders, and psychosocial problems, 2) promote health professionals use of Bright Futures guidelines and other evidence-based screening guidelines and validated screening tools in their practices, 3) support development, evaluation, and implementation of strategies that will enhance screening follow up and care coordination in the family/patient centered medical home, 4) collect, analyze, and use quantitative and qualitative data to monitor the status of screening efforts, 5) engage with relevant stakeholders to address the access and availability of early and continuous screening for medically underserved populations, and 6) promote families and health professionals involvement in public health screening efforts.
