Patient Care Coordination Demonstration Project

Description
Patient navigators working under the direction of case managers have been used by NBCCEDP grantees to mitigate barriers to screening for eligible women. These roles can and should be extended to benefit a broader population. Care models for populations under the PPACA will include the provision and coordination of primary and preventive services in settings such as Federally Qualified Health Clinics (FQHCs) and other community health systems. State Medicaid programs will cover a much larger portion of low-income women. Accordingly, this Demonstration Project (supplemental funding opportunity) will explore ways to extend proven cancer-related patient care coordination programs into new settings in preparation for impending changes in the health care delivery system.
